You may find Malaria areas in Indonesia, but not really in Bali. Therefore, no particular vaccine is required, although you can still take a preventive treatment (pills to take during the whole trip for example, and then untill 2-3 weeks after). The best is to consult a doctor that will advice you properly.
International health certificates for smallpox and cholera are only required for travellers coming from infected areas.
